Photograph of a single page from a Notice of Separation packet for an unnamed Radioman Third Class who was born in Woodstock, Virginia, on 17 December 1921.

He entered Naval service on 16 January 1945. He served in Bainbridge, Maryland, St. Louis, Missouri, and aboard the USS Aventinus.

Medals listed were: "American Area", and "Victory World War II". Both were awarded according to the "Point System".

The name, R.K. Stone, Lieutenant (jg), appears at the bottom of the page, and may be the person who prepared the form.
