Photograph of a single page from a Notice of Separation packet for an unnamed Boatswain's Mate Third Class who was born in Russell County, Virginia, on 2 May 1922.
He entered Naval service on 17 October 1942. He served in Norfolk, Virginia, the Naval Barracks in Washington D.C., and aboard the USS Potomac, USS Palawan, and the USS Cabot.
Medals listed were: American Area Campaign Medal, the World War II Victory Medal, Asiatic Pacific Area Campaign Medal, Good Conduct Medal and the Philippine Liberation Ribbon.
The name, M. Horoschak, Lieutenant (jg), appears at the bottom of the page, and may be the person who prepared the form.
