Photograph of the form titled, "Notice of the Separation From U.S. Naval Service" signed by Raymond B. Derrow, formerly of Harrisonburg, Rockingham County, Virginia, in 1945.

The form includes his duty stations (Bainbridge, Maryland, Jacksonville, Florida, and Memphis, Tennessee), his dates of service (1943-1945), and his radio operator training dates.

It also notes he was married and had worked as a leather cutter for Dally Brothers Shoe Factory in Harrisonburg before he joined the military.
