Photograph of a single page from a Notice of Separation packet for an unnamed Metalsmith Third Class who was born in Shenandoah County, Virginia, on 2 April 1927.
He entered Naval service on 28 March 1945. He served in Bainbridge, Maryland, and aboard the USS Xenia. Medals listed were: "American Area", and "Victory World War II". Both were awarded according to the "Point System".
The name, J.C. Millard, Lieutenant, appears at the bottom of the page, and may be the person who prepared the form.
