Seymour was born on September 30, 1909, in, Edinburg, Virginia.

The form includes his duty stations (Bainbridge, Maryland and Sanford, Florida), his dates of service (November 1943 - December 1945), and a description of his service.

It also noted he was single and hoped to become an electrician's apprentice and work at a lunch room and service station in Edinburg.
