Photograph of a single page from a Notice of Separation packet for an unnamed Yeoman First Class who was born in Quicksburg, Virginia, on June 23, 1910.

He entered Naval service on 3 September 1943. He served in Bainbridge, Maryland, and aboard the USS Monticillo and USS Europa.

Medals listed were: "European Theater Ribbon", "American Theater Ribbon", and "Victory Medal". .

The name, J.L. Kemmerer, Ensign, appears at the bottom of the page, and may be the person who prepared the form.
