Frederick was born on January 10, 1915, in Winchester, Virginia. He was an intern at the Medical College of Virginia in Richmond when he was commissioned as a Medical Officer in 1941.
The form includes his duty stations, (USN Hospital in Portsmouth, Virginia, Ammunition Depot in Portsmouth, and aboard the USS Roe), his dates of service, and a description of his service. When he was honorably discharged, his rank was Lieutenant JG.
It also noted he was married and hoped to have his own medical practice in Strasburg. He planned to take a correspondence course for Medical & Dental Officers of the USNR.
