Photograph of a single page from a Notice of Separation packet for an unnamed Radioman Second Class who was born in Mt. Jackson, Virginia, on 19 February 1921.
He entered Naval service on 30 December 1941. He served in several duty stations both in the U.S. and overseas.
Medals listed were: "American Theater Ribbon", "Good conduct Medal", "Commendation", "Asiatic Pacific Ribbon (6 stars)", "Philippine Liberation Ribbon", and "Victory Medal".
The name, F.R. A___ty, Lieutenant, appears at the bottom of the page, and may be the person who prepared the form.
