"This is to certify that Louise Smith, is the daughter of Mr. and Mrs. Charles Smith. And that the said Louise Smith was born on the the 10th day of March, 1913 in the town of Piedmont. County of Mineral State of West Va."
The note was notarized by J.C. Samuels on 26 September 1934.
