Photograph of a single page from a Notice of Separation packet for an unnamed "MM3c" who was born in Edinburg Virginia, on 20 January 1921.

He entered Naval service on 28 October 1943. He served in Williamburg, Virginia, Bremerton and Indian Island, Washington, Tiburon, and San Bruno, California. He also served aboard the USS San Diego.

Medals listed were: American Area Campaign Medal and the World War II Victory Medal.

The name, T.P. Shellabarger, Lieutenant (jg), appears at the bottom of the page, and may be the person who prepared the form.
