Photograph of a single page from a Notice of Separation packet for an unnamed Seaman First Class who was born in Caracas, Venezuela, S.A. on March 8, 1924.
He entered Naval service on 21 October 1943. He served in Roanoke, Virginia, Great Lakes, Illinois, Newport, R.I., Lido Beach, Long Island, New York, and the Submarine Base at New London, Connecticut.
Medals listed were: Asiatic Pacific Medal, 1 star; American Theater Medal, Victory Medal and a Submarine Combat Insignia.
The name, F.R. Ahmuty, Lieutenant, appears at the bottom of the page, and may be the person who prepared the form.
