He entered Naval service on February 1, 1944. He served in Bainbridge, Marryland and "LCS(L) (3) 58".
Medals listed were: Pacific Theater Ribbon, American theater Ribbon, Victory Medal, and Philippine Liberation Ribbon.
The name, M. E. Friot, Ensign, appears at the bottom of the page, and may be the person who prepared the form.
