Photograph of a single page from a Notice of Separation packet for an unnamed "S1c" who was born in Woodstock, Virginia, on March 25, 1926.

He entered Naval service for the second time on 24 August 1944.

He served aboard the USS Baltimore and was discharged from Bainbridge, Maryland.

Medals listed were: the Pacific Theater Ribbon, American Theater Ribbon and the Victory Medal.

The name, J.L. Kemmerer, Ensign, appears at the bottom of the page, and may be the person who prepared the form.
